CRP (C-reactive protein) and ESR (erythrocyte sedimentation rate) are markers of inflammation. They help show whether joint pain is inflammatory and are used to monitor disease activity in arthritis and vasculitis.

What the test measures

CRP is a protein made by the liver that rises quickly with inflammation. ESR measures how fast red blood cells settle in a tube, which is faster when inflammation is present.

When your doctor may order it

  • New joint pain or swelling, to check for inflammation
  • Monitoring rheumatoid arthritis, spondyloarthritis, polymyalgia or vasculitis
  • Checking response to treatment

Understanding the result

Raised CRP or ESR shows inflammation but not its cause: infections, injuries and other illnesses also raise them. ESR also rises with age and anaemia. Normal results do not always rule out inflammatory arthritis.
