HLA-B27 is a genetic marker strongly linked with ankylosing spondylitis and other forms of spondyloarthritis. It is tested when a young person has inflammatory back pain or eye inflammation (uveitis).

What the test measures

Whether you carry the HLA-B27 gene variant. Because it is genetic, the result does not change over time and the test only needs to be done once.

When your doctor may order it

  • Back pain that started before age 45, is worse with rest and better with movement
  • Morning back stiffness, or pain that wakes you in the second half of the night
  • Uveitis (painful red eye), heel pain, or psoriasis or bowel disease with joint pain

Understanding the result

Most people with ankylosing spondylitis are HLA-B27 positive, but so are a small proportion of healthy people who never develop it. A positive result supports the diagnosis only alongside symptoms and imaging; a negative result does not completely rule spondyloarthritis out.

If I am HLA-B27 positive, will I get ankylosing spondylitis?

Not necessarily. Most HLA-B27 positive people never develop it. The test is meaningful only with the right symptoms.

Do I need to repeat the HLA-B27 test?

No. It is a genetic test, so the result stays the same for life.
